
Whether in romantic relationships or marriage, separation is a reality that many of us witness throughout our lives. A breakup, betrayal, or the loss of a loved one does not only leave emotional scars—it can also have profound physical consequences. Research published over the past two decades has shown that intense emotional distress can significantly affect the body’s hormones, immune system, brain, and cardiovascular health.
In 2008, psychologists Dr. Lisa M. Diamond, Angela M. Hicks, and Kimberly D. Otter-Henderson from the University of Utah conducted a 21-day study involving 42 couples. Published in the Journal of Personality and Social Psychology, the study found that even a few days of separation significantly increased levels of cortisol, the body’s primary stress hormone. Participants also experienced sleep disturbances, anxiety, fatigue, and a range of other physical symptoms. According to the researchers, the human brain perceives close relationships as a source of safety. When that sense of security is suddenly lost, the hypothalamic-pituitary-adrenal (HPA) axis becomes activated, triggering a heightened stress response.
Many people tend to dismiss the pain of a breakup by saying, “Time heals everything.” However, science suggests that heartbreak is far from imaginary. In 2011, psychologist Dr. Ethan Kross of the University of Michigan demonstrated in a study published in the Proceedings of the National Academy of Sciences (PNAS) that recalling the experience of romantic rejection activates the same brain regions involved in processing physical pain—specifically the dorsal anterior cingulate cortex and the anterior insula. In other words, the pain of heartbreak is not merely a metaphor; it has a measurable biological basis.
Further evidence comes from a large meta-analysis published in Social Science & Medicine in 2024. After reviewing 94 studies involving approximately 1.38 million people, researchers found that relationship dissolution was associated with an increased risk of cardiovascular disease, physical illness, and diabetes in some individuals.
One of the most serious—though fortunately rare—physical complications linked to severe emotional stress is Takotsubo syndrome, commonly known as Broken Heart Syndrome. In this condition, the heart’s left ventricle temporarily weakens following intense emotional distress, leading to chest pain, shortness of breath, and electrocardiogram (ECG) changes that often resemble those seen in a heart attack.
To better understand this condition, Dr. Mohammad Reza Movahed and colleagues at the University of Arizona Sarver Heart Center analyzed data from 199,890 patients diagnosed with Takotsubo syndrome between 2016 and 2020. Their findings, published in the Journal of the American Heart Association (JAHA) in 2025, revealed that although 83% of patients were women, men had a mortality rate of 11.2%, nearly twice that of women (5.5%). The overall in-hospital mortality rate was 6.5%. While most patients recovered with appropriate treatment, the syndrome was also associated with serious complications, including cardiogenic shock, heart failure, cardiac arrhythmias, and stroke.
These findings highlight an important message: the consequences of relationship breakdown extend well beyond emotional suffering. They can also affect physical health in significant ways. Timely medical care, support from family and friends, access to mental health services, and a healthy lifestyle can all play a crucial role in reducing health risks and promoting recovery after profound emotional loss.
